| Pre-placement information: |
| Helpful information which may include a psychological or psychiatric evaluation, school records, IEP'S (for Wolfcreek Alternative School) medical records, social history, or progress (probation) summary should be mailed or faxed to: (Christian Haven's Aerie Program Director or Family Outreach Coordinator) |

| Pre-placement interview |
| Although visits to campus are always encouraged, a Christian Haven admissions representative is available to travel to your area for the pre-placement interview. Decisions regarding appropriate placements are typically made within one day of the interview. |
| Placement |
| Completion of a brief admissions packet (mostly signatures) and agreement on the date and time. Christian Haven will provide transportation for admission, if decided. |
